2/4/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home setting. Upon arrival to the home setting, Client appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C engaged mother as well as Client in a discussion to determine behaviors since the last session. Mother reports Client has been talking back to her at which mother reports behaviors last up to 20 minutes in duration. 2. Behavior: Client participated in the session without incident; however, she was agitated at times as she was discussing how she doesn’t understand the fact that her mother does not understand at times how she cannot handle certain situations due to her past and what has occurred in her life and when she cannot have her way behaviors last up to 10 minutes in duration. 1.Intervention: QMHP-C began the session by inquiring about Client’s current thoughts, feelings and emotions. QMHP-C began session by retrieving update on Client's behaviors via self-report. QMHP-C openly discussed with Client about her relationship with her mother and if there have been any improvements in communication. QMHP-C engaged Client in a therapeutic role play as well as a role play on expressing thoughts and feelings accordingly and being more assertive with those feelings without being disrespectful when explaining herself to her mother. 2.Intervention: QMHP-C provided Client with examples of how to engage her mother in conversation and discussed being receptive to criticism and feedback from her mother whenever they engage in conversation with each other. QMHP-C role-played with Client reactive and nonreactive responses as well as respecting her mother's limitations and boundaries which can be perceived as disrespectful. QMHP-C discussed her ability to follow rules and regulations set by her mother as she has been refusing to listen to directives. 1.Response: Client appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Client struggle in sessions when confronted about her behaviors toward her mother as she appears to have mixed feelings. Client has difficulty with express her feelings and thoughts as she hates her mother in one instance and loves her the next. 2.Response: Client reports that she has close relational ties with her mother sometimes which depends on their week. Through role-playing Client was able to understand reactive and non-reactive responses and learning that all feedback from her mother maybe positive/negative and that she has to take in consideration that she has to take the good with the bad.
